This is a T-shirt design over at Threadless entitled "Keeping It Real"... though, to be perfectly honest, it reminds me of this porno I might've seen once. Or twice. Or maybe it was 20 times. I forget. But, you know the one I'm talking about right? The one where the hot guy calls to get his "pipes cleaned" and winds up having some hot guy in ill-fitting overalls working away under his sink? And then things progress naturally and... oh, right, we're a family-friendly site here. I'll leave the rest of the story up to your imagination.
Anyway, the shirt: go vote for it already so we can get it made.
